To register all you need to do is approach the practice and ask the receptionist if the practice will accept you on their list.
When you initially approach the practice, if you do not have your medical card with you, we will ask you to complete a registration form and a health questionnaire detailing your past and family histories. You may be asked to provide proof of identity and residency.
PLEASE NOTE : YOU CANNOT REGISTER ONLINE
You will be offered a new patient registration medical check-up with the practice nurse; please bring a specimen of urine with you in a clean container.
